Urgent care visits are only valid at Sanitas Medical Center in your state of membership (Florida). |
| Behavioral Health |
One (1) initial consultation and one (1) follow-up visit with a clinical social worker for mild to moderate behavioral health issues (such as mild to moderate anxiety and/or depression) per calendar year. Behavioral Health services require a referral by a Sanitas PCP as the result of a PCP visit at a Sanitas Medical Center. |

| Pharmacy |
Access to an online pharmacy that searches for discounted prices on drugs near you. Select your medication from your preferred pharmacy or get it delivered to your home for no additional cost. The online pharmacy is provided in alliance with the cloud pharmacy Blink Health. On-site medication dispensing is available at select Sanitas Medical Centers in Florida at competitive self-pay prices. The cost of medications is not included in your membership fees. |

***We may periodically change the terms of the SanitasCare Direct Primary Care (DPC) Program to comply with applicable laws (that change from time to time), and/or to otherwise ensure the viability of the DPC Program so that the valuable services continue to be available to our members. If we deem any changes to this Agreement necessary, such as adjustments to the included services and/or membership fees, we will provide you with written notice at least 60 calendars day prior to the effective date of any material change(s). If you disagree with any changes, you have the right to terminate your membership by providing 30 days’ notice.
