Checklist before your visit
  • Bring your photo ID and if you have insurance your insurance card.
  • Write down all of your healthcare concerns and questions you’d like to discuss with your doctor during your visit.
  • Be prepared to provide your medical history including dates of any prior hospitalizations or surgeries and any diseases that run in your family.
  • Bring all of your medications in their original containers to your appointment, or bring a list of medications including the names, dosages and how they are taken.
  • Bring your immunizations records, and if possible your medical records and past test results from other providers.
  • If you are a new patient, complete the registration forms prior to the visit to save time.

During your visit

  • Go over your list of questions or concerns with your doctor and take notes. This will help you to remember everything your doctor says, and to follow your doctor’s instructions.
  • Discuss your symptoms if you have a health problem, and any recent changes you may have noticed.
  • Answer all of your doctor’s questions. Be honest about your diet and sexual history, how much you exercise and whether you smoke, drink alcohol or use drugs.
  • Describe any allergies to medications, foods, pollen or other things. Also, tell your doctor about your family’s health history.
  • Tell your doctor if you may be pregnant or if you are trying to get pregnant. Some medications are not safe if you are pregnant or about to become pregnant.
  • Ask all questions about your condition and treatment, and any specific things you can do to ease your symptoms.
  • Talk with your doctor about all new medications and how to take them.
  • Ask questions if you don’t understand something. If you’re not sure you understood, your doctor would gladly explain it again.

The patient registration form contains very basic information about the patient and medical history. This information includes the contact information, insurance and billing information, consent to treatment and privacy notices.

Medical Records Release
To maintain patient confidentiality and assure compliance with federal and state privacy laws, health information may not be released without a patient’s written authorization (except as permitted by law). To request your health records, you will need to download, complete and sign this medical release form.

Health Information Exchange Opt-out
Health Information Exchanges (HIE) allow your medical information to be available and viewed electronically by your Sanitas provider and care team members.
Chest X-ray Patient Guide
Chest X-rays produce images of your heart, lungs, blood vessels, airways, and the bones of your chest and spine. The image helps your doctor determine whether you have heart problems, a collapsed lung, pneumonia, broken ribs, emphysema, cancer or any of several other conditions.
Pelvis X-ray Patient Guide
A skull x-ray is a picture of the bones surrounding the brain, including the facial bones, the nose, and the sinuses. Your doctor may order this x-ray if you have injured your skull. You may also have this x-ray if you have symptoms or signs of a structural problem inside the skull, such as a tumor or bleeding.

Spinal X-ray Patient Guide
Spinal x-rays are pictures of the spine. They may be taken to check the curve of your spine or to find injuries or diseases that affect the discs or joints in your spine. These problems may include spinal fractures, infections, dislocations, tumors, bone spurs, or disc disease.
Upper Extremity X-ray Patient Guide
An upper extremity x-ray is a test that uses radiation to produce detailed images of the bones of the upper extremity, which includes the fingers, hand, wrist, elbow, forearm, upper arm and shoulder. The image is used to various injuries and conditions such as arthritis, unexplained swelling or pain, bone deformities, infections and tumors.
Lower Extremity X-ray Patient Guide
A lower extremity x-ray is a test that uses radiation to produce detailed images of the toes, feet, ankles, lower leg, knee, upper leg or hip. The image is used to detect conditions such as fractures, soft tissue damage and arthritis, or when you have an injury to a lower extremity or you experience unexplained swelling or pain in the area.
Utrasound Patient Guide
An ultrasound is an imaging method that uses high-frequency sound waves to produce images of structures within your body. The images can provide valuable information for diagnosing and treating a variety of diseases and conditions.
CT-Scan Patient Guide
A computerized tomography (CT) scan combines a series of X-ray images taken from different angles to create more detailed images of the bones, blood vessels and soft tissues inside your body. The images can provide valuable information for diagnosing and treating a variety of diseases and conditions.
Mammography Patient Guide
A mammogram is an X-ray image of your breasts used to screen for breast cancer. Mammograms play a key role in early breast cancer detection and help decrease breast cancer deaths.
Colposcopy Patient Guide
Your doctor may recommend colposcopy if your pap test has shown abnormal results. If your doctor finds an unusual area of cells during colposcopy, a sample of tissue can be collected for laboratory testing (biopsy).
Holter Monitor Test Patient Guide
A Holter monitor is a small, wearable device that keeps track of your heart rhythm. During that time, the device records all of your heartbeats.
Stress Test Patient Guide
A stress test gathers information about how your heart works during physical activity. It involves walking on a treadmill while your heart rhythm, blood pressure and breathing are monitored.
Colonoscopy Patient Guide
A colonoscopy is an exam used to detect changes or abnormalities in the large intestine (colon) and rectum. During a colonoscopy, a long, flexible tube with a tiny camera is inserted into the rectum to allow your doctor to view the inside of the entire colon.
Bowel Preparation Patient Guide
This guide will help you prepare before a colonoscopy. You'll need to clean out (empty) your colon. Any residue in your colon may obscure the view of your colon and rectum during the exam.

  • View your personal medical record and update your information
  • Browse and read educational materials on a wide range of health topics
  • Share secure messages with the staff in your Sanitas medical center
  • Receive reminders about upcoming appointments, events and preparation for tests and procedures
  • View referrals to specialists and request new ones

How to access your patient account

  •     Once your Patient Portal is activated, you will receive an email from Sanitas. The email will contain your patient portal username, password and the patient portal URL link.
  •     Click the link in the email to launch the patient portal
  •     Enter the user name and password provided to you in the email. Click login
  •     The user validation screen will display. Enter your date of birth or phone number. Click submit.
  •     You will be required to enter a new password and select a security question. When complete, click confirm.
  •     The next window requires you to provide Consents. Read the eClinicalWorksConsent Form. Click Next.
  •     Read the practice consent form. Check the box “I have read the consent form and the above information”. Click submit.
  •     A small window will ask you to confirm. Click ok.
  •     The patient portal screen will display.
  •     Categories appear along the left hand column of the window.
  •     Click the home image to return to the screen at any time.

 

An email from the Patient Portal will be sent to you when new messages or updates to your medical record are posted to the portal. You MUST log in to the portal to review. No medical information will appear in an email.

Yes. You can bring family members with you for your own medical appointments. Your family members (kids, adults and seniors) are also welcome to become patients.
Be sure to bring a complete list of the medications and dosages you currently are taking. Your doctor also may ask to see your medications, so please have them available. Please also bring a copy of your medical record, if available, and any lab, imaging or diagnostic test results related to your current medical concerns. In Florida, we accept Florida Blue and original Medicare. We also accept self-pay patients.
Some specialty areas at Sanitas require a doctor's referral, and some do not. It varies based on the type of insurance plan you have. Call us at 1-844-665-4827 to confirm if a referral is needed. Since Sanitas has most types of specialists on staff in our medical centers, it’s easy to get an appointment once the paperwork (if needed) is complete.
